Nurse Practitioner - Youth Health (2-3 days/w)

Are you an experienced Nurse Practitioner who is passionate about supporting rangatahi to thrive? Evolve is looking a NP to join our primary health care team in a busy Youth One-Stop shop. 

To be able to do this role, youll need sound clinical experience and be registered as a Nurse Practitioner with current practicing certificate. Ideally you have experience working in the areas of Youth Health, Mental Health and Sexual Health.

No two days are the same, so youll need flexibility under pressure and the ability to find innovative solutions in complex situations. Youll be effective in managing your own time and workload, be self-directed, meet deadlines and maintain strict confidentiality. But you wouldnt be doing this alone, youd be part of a positive, multi-disciplinary team, recognising strengths and opportunities to better support rangatahi in Te Whanganui-a-Tara.

You must have a demonstrated commitment to working within the Treaty of Waitangi and understanding of partnership.

Evolve is a charitable trust started 18 years ago by an amazing group of young people who saw a need for a service that addressed the specific needs and desires of young people in Wellington and was designed and run by young people. Since then we have become a leader in delivering innovative services. 

Evolve is a ‘one stop shop youth service providing health care and social support to young people aged 10-24 years. Our wraparound model allows us to address health needs while also addressing social and developmental issues which are often the underlying determinants of ill health and poor outcomes for young people. 

We work from a youth development model that emphasises the importance of the connections of young people to their communities, families, schools and peers, and has a focus on fostering resilience and supporting young people to make positive decisions. Evolves multi-disciplinary team (MDT) includes Nurses, Doctors, Counsellors, Youth Workers and our Kaitautoko-i-te-Ora. The Evolve team works to reduce barriers so that young people are able to access the support they need.
