We are recruiting for a temporary Disability Outcomes Coach to support our client, a well-established health and disability services provider based in Mt Eden. This is a 1–3 month assignment starting as soon as possible, offering a mix of office-based and community-based work in a fast-paced and rewarding environment.

This role plays an integral part in ensuring high-quality outcomes for people with disabilities and their families, particularly during periods of change, stress, or crisis.

About the Role

In this role, you will work closely with clients who receive Individualised Funding and Younger Persons with Disabilities funding. This funding supports services such as home support, respite, mobility aids, sensory equipment, and other disability-related needs.

The role involves significant coordination, attention to detail, and relationship management to ensure funding is being used appropriately and in line with NASC and Ministry of Social Development criteria.

Key Responsibilities

  • Assist in managing the quality of services provided to clients with disabilities by completing annual reviews with clients and their families
  • Develop and maintain effective, professional relationships with clients and whānau to ensure a high standard of service
  • Coach and support clients and families to make the best use of their funding, 
  • Monitor and review how clients are using their individualised funding against approved criteria
  • Provide practical support, including checking that clients’ employees (support workers/carers) are paid correctly and on time
  • Maintain accurate and up-to-date records using the client management system
  • Demonstrate understanding of the position of Māori as tangata whenua, and show commitment to the principles of Te Tiriti o Waitangi
  • Promote a strong health and safety culture through planning, delivery, monitoring, and review of health and safety practices

Skills and Experience Required

  • Experience delivering client-focused or care-based services
  • A genuine commitment to supporting disabled people to live well and independently in the community
  • Experience working within disability services is preferred and considered an advantage
  • Strong listening, verbal communication, and interpersonal skills, particularly when working with people experiencing stress or crisis
  • High level of attention to detail and confidence navigating funding criteria and guidelines
  • Competent computer skills and confidence using database or client management systems (desirable)
  • Ability to prioritise workload and ensure client reviews are completed within required timeframes
  • A collaborative team player who contributes positively and manages conflict professionally
