Active Families Coordinator

Full-time 40 hours (includes 10 hours Green Prescription) Responsible to: Healthy Lifestyles Manager (HLM) Functional Relationship with: All other Sport Otago Staff

Key Relationship with: Clients and their family/whānau, Primary Care Practices, General Practitioners (GPs) and Practice Nurses (PNs), Non- Governmental Organisations (NGOs), education providers, social service agencies, recreational facilities, community activity providers, and the wider health and physical activity sectors.

Purpose of Position Active Families is a community-based health intervention programme, which aims to educate and encourage inactive children and their families to achieve sustainable lifestyle changes in the areas of physical activity, nutrition, and behaviour change. The Active Families Coordinator is responsible for the effective delivery, monitoring, and evaluation of the Active Families programme and for providing the appropriate prescription of exercise, nutrition information, and other health advice to all referred whānau. The Active Families Coordinator is also responsible for delivering 10 hours per week of the Green Prescription (GRx) programme. GRx supports adults aged 17+ who have been referred to the programme to build their own skills, knowledge, and confidence to manage their health and wellbeing. The roles responsibility is to prescribe physical activity, nutrition, and lifestyle intervention appropriate for the clients ability, long-term conditions, and goals.

KEY TASKS OUTCOMES

Active Families Programme Implement an intervention programme for families to improve physical activity and healthy eating behaviour.

Provide ongoing support to Active Families participants, through in- home delivery, phone, text, video, or email support.

GRx Programme In accordance with the Ministry of Health GRx guidelines, dispense individual and risk stratified physical activity advice appropriate to age, fitness levels, abilities, medical conditions, and home environment.

Provide regular GRx client support via telephone, email, or video call to GRx clients based in regional Otago.

Provide regular GRx client support as appropriate to parents of children referred to the Active Families programme.

Relationship Management Work collaboratively with the HLM to maintain and strengthen networks and effective working relationships with the health and education sectors, social service agencies, recreational facilities, and community activity providers.

Contribute to creating positive awareness of Sport Otagos health programmes and physical activity opportunities within the community.

Health Promotion In conjunction with the HLM, develop and keep current information

and contact details for GPs, PNs, and other health professionals in the region.

Liaise with the HLM to develop and implement effective

promotional strategies to grow the awareness and utilisation of the

GRx programme by GPs, PNs, and other health professionals.

Liaise with the HLM to develop and implement effective promotional strategies to grow the awareness and utilisation of the Active Families programme.

Programme Reporting and Assist in the implementation of effective performance measurement

Accountability processes to evaluate the impact of the Active Families and GRx

programmes.

Ensure that all reporting to measure progress and impact is

completed on time and is compliant with Sport Otagos reporting protocols.

Maintain appropriate client contact records on the CRM.

Supply appropriate client progress reports to Active Families and

GRx referrers in accordance with Ministry of Health guidelines.

Best Practice Demonstrate and implement best practice processes and

procedures for Sport Otagos Healthy Lifestyles programmes and

initiatives.

Ensure the Treaty of Waitangi and its principles of Partnership, Protection, and Participation are acknowledged and reflected in the implementation of the Healthy Lifestyles programmes and initiatives. Ensure all decisions and behaviours are inclusive, embrace diversity, and demonstrate a commitment to social justice, equity, and equality. Mitigate and openly communicate any possible risks or incidents that have the potential to bring Sport Otago and/or invested partners into disrepute. Actively collaborate with the wider Sport Otago team, establishing a good working knowledge of other programmes and initiatives, and adding value to these workstreams via the integration of Healthy Active Lifestyles programmes and initiatives.

Person Specification

Relevant qualification and/or experience in the prescription of exercise to improve health and wellbeing.

Sound working knowledge of health and exercise psychology.

An understanding of the physical activity and recreation sectors, and awareness and knowledge of community-based physical activity providers.

Knowledge of the systems, structures, and operating models that underpin the health sector across Aotearoa.

Knowledge of, and commitment to, the Treaty of Waitangi.

A positive disposition towards physical activity and the importance and value it has on the health and wellbeing of New Zealanders.

Working knowledge of behavioural change practices.

The ability to motivate and support both individuals and whānau with empathy and understanding.

Exceptional relationship building skills, accompanied by a high level of emotional and social intelligence.

A high level of verbal and written communication skills, with the ability to engage and communicate effectively to a wide range of stakeholders and audiences.

A track record of handling confidential and sensitive matters with exemplary discretion and professionalism.

Awareness of the wider social and political context in which Regional Sport Trusts operate.

Fosters a team spirit through a collaborative and cooperative approach to work.
